ADHC Post-School Programs
There are two post-school programs offered by the NSW Department of Human Services Ageing, Disability and Home Care (ADHC) to people with a disability who complete Year 12.
Community Participation
Community Participation (CP) aims to assist young people with a disability to develop the skills they need to work towards their goals, increase their independence and participate as valued and active members of the community. CP has been developed for young people with a disability with moderate to high support needs who require an alternative to paid employment or further education in the medium or longer term. It is usually provided in a group environment.
Post-school eligibility assessments are conducted in the final year of school by specially trained teachers. Eligible students are notified by ADHC in October of the final year and referred to providers of these services.
Transition to Work program
Transition to Work (TTW) is a two-year program with the aim of achieving employment for young people with a disability. The program develops the skills and/or qualifications needed by young people to successfully secure and sustain employment. TTW provides services to young people with a range of support needs but is primarily targeted to young people with moderate to high needs who are unable to immediately access employment or attend TAFE or university due to their support needs.
Post-school eligibility assessments are conducted in the final year of school by specially trained teachers. Eligible students are notified by ADHC in October of the final year and referred to providers of these services.
Applicants need to have an assessment completed to ensure that they are referred to the program most suited to their skills and needs. This assessment takes place while the student is still at school and is completed in the first half of Year 12.
Post school program eligibility
To be able to access a Post School Program, you need to
- live in NSW
- have a disability as defined by the Disability Services Act 1993
- have completed year 12.

- ADHC writes to all applicants and their families about:

The letter will also contain information about the appeals process.
Eligible school leavers and their families will also be provided with information about choosing a Transition to Work or Community Participation service provider, including information on providers.
If the young person is not eligible for a ADHC post -school program, information will be provided about programs provided by other agencies.
